Mildronate is a structural analogue of carnitine, betaine could competitively inhibit acid hydroxylase, thereby inhibiting the biosynthesis of carnitine, direct inhibition of carnitine-dependent fatty acid transport in mitochondria. Inhibition of the biosynthesis of carnitine may reduce intracellular concentrations of free carnitine, prevent isoproterenol-induced accumulation of acyl carnitine. Therefore, this drug has significant myocardial protection role in this protective effect while producing no significant effect on the hemodynamic parameters, blood supply to the heart and cardiac oxygen consumption did not change.
